Diagnosis of Acute Appendicitis Using Ultrasonography



Abstract
It is well known that acute appendicitis of the most common surgical disease of acute abdomen. For the evaluation of acute appendicits many diagnostic methods, such as physical examination, laboratory findings, palin abdomen and barium enema,
have
been
widely used. But the diagnostic accuracy is still 70 to 82%%, and moreover 54 to 65% in young women.
The purpose of this study was to improve the diagnostic accuracy and to make early diagnosis of acute appendicitis using ultrasonography. High-resolution real-time ultrasonography was performed in 83 patients with clinically suspected acute
appendicitis
for 13 months. Ultrasonographic findings, operative findings and results fo clinical follow-up acute appendicitis were analysed and corrected.
@ES The results were as follows;
@EN 1) the sex distribution of cases were 33 males and 50 females, and most common in the second decade(26.5%).
2) The positive ultrasonograhic findings were in 31 cases(37.3%), those were acute appendicitis in 23 cases(74.2%), mesenteric lymphadenitis in 4 cases(12.9%), clinical follow-up without operation in 4 cases(12.9%).
3) The ultrasonographic findings of normal appendix were in 38 cases(45.8%), but 13 cases(34.2%) were done operation, 8 cses(21.0%) were acute appendicitis, 5 cases(13.2%) were measenteric lymphadenitis postoperatively.
4) The ultrasonographic findings of the fluid collection within the cul-de-sac were 6 cases, the right ovarian cyst were 8 cases. The 14 cases(16.9) were clinical follow-up and transfer to department of OB/GY without operation.
5) The overall accuracy was 80.7% with 84.6% of sensitivity ad 74.2 of specificity.
